Section 1 - VERIFICATION

INCIDENT EXCLUSIONS: if you have more than two (2) rooms and water level is greater than two (2) Inches the condition of the property is not eligible.

Section 2 - CONFIRMATION

You must satisfy one of the financial qualifying criteria below to be eligible for the Program.

Please make a selection by clicking the left option of the category that most closely relates to your status.

I am retired, 65 years or older, and on a limited or fixed yearly (gross) household income of less than $65,000.

I am a veteran and/or have a disability with a yearly (gross) household income of less than $65,000.

I am employed, and my yearly household income is: $18,000 Maximum for a household of (1) person; $24,000 Maximum for a household of (2) persons; $35,000 Maximum for a household of (3) persons; $47,000 Maximum for a household of (4) persons; $65,000 Maximum for a household of (5) or more persons.

I participate in at least one of the following programs: Supplemental Security Income (SSI), The Special Supplemental Nutrition Program for Women, Infants, and Children (WIC), Supplemental Nutrition Assistance Program (SNAP),Medicaid, Medicare, Social Security Disability Insurance (SSDI) or a similar income restricted program.

I have homeowners or renters insurance or another form of relief

None of the eligibility criteria apply.
